LIVE WELL - Chronic disease prevention and management program

Live Well is a 24-week health promotion program designed for adults living with one or more chronic conditions. Live Well aims to support participants in building self-management skills and sustaining holistic health behaviour changes over time. It is structured around four flexible, six-week pillars that participants can complete in any order, based on their needs and readiness.

  • KNOW Well - focuses on self-management skills and is delivered in partnership with Self-Management BC’s Chronic Conditions program. This evidence-based model has been extensively evaluated and shown to improve self-efficacy, reduce emergency room visits, and enhance quality of life.

  • BE Well - our upcoming pillar focused on stress and emotional wellbeing, will integrate content from the University of Ottawa Heart Institute’s Stress Management Program, drawing on evidence-informed strategies to support participants in managing stress and fostering resilience.

  • MOVE Well - was developed in-house by REACH’s public health kinesiologist, using best practices in physical activity promotion. The series supports participants in finding accessible, progressive, and enjoyable ways to incorporate movement into their lives, tailored to varying abilities, goals, and comfort levels.

  • EAT Well - is delivered in partnership with the BCCDC’s Food Skills for Families program. Program evaluations show that participants report eating more fruits and vegetables, a stronger understanding of healthy eating, cooking more meals from scratch, and feeling more confident in the kitchen.
