Medical Office Assistant
Department: Medical Clinic / Permanent part-time (0.6 FTE or 24 hours/week)
The MOA contributes to all aspects of clinic operations and patient care, providing detailed and pro-active administrative and clinical support to patients and colleagues in a team-based environment. The MOA is committed to supporting the provision of longitudinal primary care to patients with complex health and social needs, and approaches this task with empathy and cultural humility.
Nurse Practitioner
Department: Urgent and Primary Care Centre (UPCC) / Locum positions
The Nurse Practitioner (NP) will provide urgent primary care within a team-based care model. Nurse Practitioners will be expected to manage the entire range of acute primary care problems usually presenting to a family physicians office, including minor procedures such as suturing wounds, splinting injuries and responding to acute psychiatric or substance withdrawal scenarios. They will be comfortable with resuscitating and stabilizing a patient whose acute medical needs exceed the capability of the unit while awaiting transfer to an emergency room.
Department: Medical Clinic / Part-time (0.8 FTE or 28 hours per week)
Nurse Practitioners working in the medical clinic will manage the entire range of acute primary care problems usually presenting to a family physicians office. Nurse Practitioners, along with physicians will lead and empower the team. They will help plan service delivery to ensure work is satisfying and effective from the perspective of the Practitioner, the participants and the organization. The NP will provide client and family-centered care through a trauma-informed approach and evidence-informed practice.
Nurse Practitioner Post Graduate Advanced Education (pgae)
Department: Urgent and Primary Care Centre (UPCC) / Part time or Full time (28-35 hours per week)
REACH UPCC is now offering a new and exciting 6 months Nurse Practitioner Post Graduate Advanced Education Program. This is your opportunity to consolidate your Nurse Practitioner education with a team helping to support your successful transition into independent practice.
PHYSICIAN - FAmily physician/general practitioner
Department: Medical / Part time, Full time, Locum positions
The Physician will show a high level of clinical skill and competence in combination with exceptional communication skills with team members and patients who may be vulnerable for complex medical or social reasons. They are willing and able to participate as a prescriber of Opioid Agonist Therapy integrated with primary care to patients where needed or will obtain this competency shortly after joining the service. They will have a good understanding of, and commitment to addressing the social determinants of health in their contact with patients and in their interactions with the wider team.

Registered Nurse
Department: Urgent and Primary Care Centre (UPCC) / Casual
The Registered Nurse works at the full extent of their scope of practice in a primary care setting with a focus on urgent care. The RN performs a critical role in an interdisciplinary team triaging all patients and directing to appropriate care, whether that is within the REACH UPCC, or elsewhere. There are also opportunities for managing a patients’ presenting issue independently or via protocols or in consultation with other team members such as physicians or nurse practitioners.
Department: Medical / Permanent Part-time (0.8 FTE or 28 hours/week)
The primary care nurse cares for families and individuals across the lifespan. This includes health promotion, screening and assessment, counselling and education, and treatment and for minor and acute illnesses, and chronic care management within the nursing scope of practice.

Board of Directors: REACH Community Health Centre is led by a volunteer Board of Directors and we are actively recruiting candidates in advance of our next Annual General Meeting. Their role is to govern the organization on behalf of the community, in a timely, transparent, and accountable manner by overseeing the performance of the organization and ensuring fiscal integrity and regulatory compliance.

REACH will assist any employee or family member who loses their primary medical care as a result of this policy to obtain comparable medical care as soon as possible after the conclusion of the probationary period.
