Clinical Counselor or social worker for link to employment

Department: Health Equity and Engagement Department (HEED) / 21-35 hours per week

REACH is recruiting a part-time Clinical Counsellor and /or Master degree Social Worker or equivalent to work collaboratively with the LINK to Employment Program and with the multi-disciplinary REACH care team, providing comprehensive primary health care for patients with complex health and social needs. Working both independently and collaboratively with individuals, staff, community partners and volunteers, the RCC/SW will facilitate and deliver a variety of services including but not limited to short-term counseling, group work, system navigation advocacy, education and outreach programming.

dENTal hygienist

Department: Dental / Full time

The dental hygienist provides preventative and hygiene services as an integral part of the overall treatment plan for a patient as discussed with the dentist to promote and maintain dental and oral health. Responsibilities include contributing to the overall running of the clinic, input into policy/protocol development and helping to maintain all clinic standards. We are looking for an experienced, detail-oriented and organized team player.

lICENSED pRACTICAL nURSE

Department: Medical / Part time (14 hours/week)

The LPN performs job duties through a trauma informed care framework and culturally sensitive and safe lens for families and individuals. They will work closely with the operations manager to ensure the smooth and efficient operation of the medical clinic. This includes client flow at all stages from entry to exit to follow up, including intake, charts, lab tests, referrals and communication between the medical reception and the care providers.

Medical Office Assistant

Department: Medical Clinic/ Part time 0.8FTE (28 hours/week), Full time also available

Reporting to the Operations Manager, the MOA contributes to all aspects of clinic operations and patient care, providing detailed and pro-active administrative and clinical support to patients and colleagues in a team-based environment.  Working to full scope, the MOA is committed to supporting the provision of longitudinal primary care to patients with complex health and social needs, and approaches this task with empathy and cultural humility.  The clinic is open extended hours and the post-holder may be required, in cooperation with other MOAs, to work on weekends and evenings, and in rare instances, provide short notice cover for sickness absence.

Department: UPCC/ Part time 0.6FTE (21 hours/week)

This position provides team-based care and works to full scope as an MOA.  The MOA performs job duties through a trauma informed care framework and culturally sensitive and safe lens for families and individuals.  Reporting to the Operations Manager, the MOA contributes to the operations of the UPCC through administrative and clinical support to our care providers in a team-based environment.  Together with the interprofessional team they will play an important role in assessing urgency, and deciding on who the most appropriate team member is to meet their needs. The MOA will have the opportunity to work flexible hours on evenings and weekends.

Nurse Practitioner

Department: Urgent and Primary Care Centre (UPCC) / Full time (35 hours per week)

The Nurse Practitioner (NP) will provide urgent primary care within a team-based care model. Nurse Practitioners will be expected to manage the entire range of acute primary care problems usually presenting to a family physicians office, including minor procedures such as suturing wounds, splinting injuries and responding to acute psychiatric or substance withdrawal scenarios. They will be comfortable with resuscitating and stabilizing a patient whose acute medical needs exceed the capability of the unit while awaiting transfer to an emergency room.

Nurse Practitioners working in the medical clinic will manage the entire range of acute primary care problems usually presenting to a family physicians office. Nurse Practitioners, along with physicians will lead and empower the team.  They will help plan service delivery to ensure work is satisfying and effective from the perspective of the Practitioner, the participants and the organization. The NP will provide client and family-centered care through a trauma-informed approach and evidence-informed practice.

The RN-C cares for families and individuals across the lifespan. This includes health promotion, screening and assessment, counselling and education, and treatment and for minor and acute illnesses, and chronic care management within the nursing scope of practice.

PHYSICIAN - FAmily physician/general practitioner

Department: Medical / Part time to Full time available

The Physician will show a high level of clinical skill and competence in combination with exceptional communication skills with team members and patients who may be vulnerable for complex medical or social reasons. They are willing and able to participate as a prescriber of Opioid Agonist Therapy integrated with primary care to patients where needed or will obtain this competency shortly after joining the service. They will have a good understanding of, and commitment to addressing the social determinants of health in their contact with patients and in their interactions with the wider team.

Social Worker

Department: Medical / Part time (21 hours/week)

The social worker is responsible for advanced clinical and consultative social work services to clients and families focusing on how the client’s relationships are affected by their environment.  Duties include working in partnership with the care team to offer holistic patient care which includes conducting social and psychosocial assessments, improving access to community services and resources and initiatives to reduce inequities, providing short term counseling interventions, maintaining client records, attending multidisciplinary clinical/case meetings and conferences, identifying applicable community resources for clients and facilitating care throughout the continuum.

REACH’s Basics for Health Program: This volunteer led program helps navigating people in our community to improve health outcomes. Perfect for students who are in health care programs to gain meaningful and valuable healthcare work experience.

Board of Directors: REACH Community Health Centre is led by a volunteer Board of Directors and we are actively recruiting candidates in advance of our next Annual General Meeting. Their role is to govern the organization on behalf of the community, in a timely, transparent, and accountable manner by overseeing the performance of the organization and ensuring fiscal integrity and regulatory compliance.
